Central Southern Tier Health Care Plan

The Central Southern Tier Health Care Plan administers the health care benefits for eight school districts and the GST BOCES. The covered benefits under this Plan are self funded. The Third Party Administrators are RMSCO for the medical/hospital, MEDCO for the prescription drugs, and Corporate Care for our Managed Care.

INSURANCE SUMMARY FOR GST BOCES EMPLOYEES AND RETIREES

This Plan includes the PPO Network (provider network information can be found at http://www.rmscoinc.com) and the following endorsements:
$10.00 Office Visit Co-Pay
$50.00 Emergency Room Co-pay for NON Emergencies
Three Tier Formulary Prescription Drug Program - $5.00 Co-pay on Generic Drugs, $15.00 Co-Pay on Preferred (brand name) drugs, and $40.00 Co-pay on Non-preferred drugs at BOTH retail (local) Pharmacy and Mail Order.

COBRA Regulations

If you don’t notify the Plan within 60 days of a qualifying event, a person could lose their rights under COBRA to carry their insurance at our group rate
Examples of qualifying events are divorce, dependent students ceasing to be dependents, child marrying, etc

HIPAA Regulations

Allows a person to have pre-existing condition clause waived or open enrollment period waived if they have had 18 continuous months of insurance coverage prior to employment
Central Southern Tier Health Care Plan HAS established a policy to guard against unnecessary disclosure of your health information
Authorization to Designate a Personal Representative Form needs to be completed and returned to RMSCO so that a member may access sensitive information regarding their dependents claims or benefits. Form may be found at http://www.rmscoinc.com
Please remember that due to the new HIPAA regulations, we are restricted to the assistance that we can provide due to the federal mandates on your PHI (Protected Health information)

Enrollment Changes

Documentation may be required when you add any dependents to your coverage pursuant to the Plan document
Failure to notify the Plan when a dependent becomes ineligible for coverage could result in the dependent losing their continuation of coverage under COBRA AND
Would make you responsible to reimburse the Plan of any costs incurred retro to the termination date

Dependent Child Reminder

PLEASE REMEMBER THAT WHEN YOUR CHILD IS NO LONGER ELIGIBLE AS A DEPENDENT, IT IS YOUR RESPONSIBILITY TO NOTIFY RMSCO TO AVID LOSING THEIR RIGHTS TO COINTUATION OF COVERAGE UNDER COBRA.
